Haven't done vs on any custom campaigns to be honest, always figured there was a lack of interest, not to mention most official servers and our own don't support them.
A handful are worth trying; the Lef5 4 Dead blog used to have a list of featured maps. I remember a couple of them, but the blog no longer seems to have the list:

Helm's Deep is amazing.
City 17 is amazing.
Haunted Forest is ok.
Suicide Blitz is even worse than Cold Stream beta.
Not familiar with Death Aboard.

From a gameplay standpoint, these two aren't the best, but I really enjoyed them because of the theming:
Zombies of the Caribbean
Indiana Jones and the Temple of Zombies
They're both by the same guy, and both are based on Disneyland rides - Pirates of the Caribbean and the Indiana Jones Adventure, specifically. The Indy one is far more polished and uses a lot of custom content to do a remarkable job of fully re-creating the ride, whereas the Pirates one has a small amount of custom content but uses default Infected models for all the ride's characters, which looks rather awkward. There is also a bug that I found in the Pirates one where occasionally the campaign cannot be completed because the button to open a gate near the end fails to work. I've only seen that happen once though, so it may have just been a fluke.
Like I said, neither one is great from a gameplay standpoint, but if anyone's ever looking for a novelty campaign to play without any concern for seriousness, polish (in the PotC one at least, although the Indy one also has a handful of rare game-breakers if you do things wrong), or challenge (both campaigns are quite easy and very short), I think they're at least worth checking out.

For a quick synopsis: It's basically Half-Life 2 Episode 1, if Half-Life 2 Episode 1 were Left 4 Dead 2. And it works so well.

I did a single player playthrough of the workshop's version of City 17, I'm actually going to recommend against playing it sometime. I don't know if it was a combination of Advanced and the Director being a $&#%! but there was next to nothing in terms of supplies, leading to me having to pace myself very very slowly throughout the campaign so that I wouldn't burn what little I was finding. In what may very well be some sort of epic fail, I found more defibrillators than pain pills, despite not a single death until the finale.
The campaign is also extremely long. Start to finish clocked a full hour, which is average for a VS match. It's just not as good as I remember it.
On a side note, Helm's Deep is a Survival map; we can't play VS on it without the Survival VS mutation in place. Helm's Deep however is actually loaded on Valve's official L4D2 servers (the workshop version) so you don't need to worry about loading it onto ours; if we don't already have it.
