On the endmap splash screen giving the map stats, my player name has a box with a question mark. I see other folks have some sort of avatar - where is it that I designate some image as my avatar? I have one in mind that's about the right size, but I have no clue as to where to select it in the DoD:S menu.

You can set it in your Steam profile.
From the main Steam window -> Community tab -> Edit my profile (right sidebar) -> Avatar
